We are creating a software network that empowers healthcare professionals serving the aging population, particularly in home health and post-acute settings. Our focus is on coordinating analytics-driven workflows across organizational boundaries. For example, an elderly patient who undergoes surgery at a hospital for joint replacement or heart failure will require continued care by a home health agency or skilled nursing facility. Coordination between providers is done mainly through phone and fax and marked by incorrect treatments and medications, unnecessary hospital readmissions, and inefficiencies in care and billable activities. This problem is growing as there will be 80 million baby boomers joining Medicare over the next 20 years. Because of this trend, home health care is projected to be the fastest-growing industry in the country. Our target customers include home health agencies, skilled nursing facilities, physician groups, and health systems.
