WHO WE ARE CenterLight Health System is a leading non-profit provider of health and related services to older adults in need across the New York metropolitan area. Our subsidiaries include CenterLight Healthcare, an innovative managed long-term care company; Best Choice, a licensed home care services agency; Direct, an Institutional Special Needs Plan; and senior housing. Teamcare, CenterLight Healthcare's flagship Program of All-Inclusive Care for the Elderly (PACE), is one of the oldest and largest such programs in the United States. WHO WE SERVE CenterLight Teamcare is a Medicare-Medicaid plan for participants aged 55 years and older, who require long-term care. CenterLight Teamcare serves participants from a numerous backgrounds and languages including Arabic, Albanian, Chinese, Creole, English, Korean, Hindi, Punjabi, Russian and Spanish. WHERE WE OPERATE We operate 14 locations throughout the five boroughs of New York City, Westchester and Suffolk counties. A TEAM-BASED APPROACH TO CARE CenterLight Teamcare develops individualized care plans, which are carried out by a dedicated interdisciplinary team (IDT) of doctors, nurses, physical therapists, nutritionists, recreational therapists and other professionals working in concert to provide care as unique as the richly diverse participants enrolled in the plan. CenterLight recently rebranded its PACE plan to highlight the strength of its interdisciplinary team in providing exceptional quality care to those we serve.
