Transformation Partners in Health and Care (TPHC) offer high-quality bespoke consultancy services, and support to programmes and projects from neighbourhood to national levels, working with a range of partners from integrated care systems, local government, and NHS organisations. TPHC has an outstanding track record in achieving positive and tangible results for populations. In late 2022 we changed our name when our two teams - Healthy London Partnership and Healthcare Consulting came together as a single team called 'Transformation Partners in Health and Care'.
