Thresholds Health will be a full-service primary care health center that will welcome all members of communities on the west side of Chicago, regardless of insurance status or ability to pay, as well as existing Thresholds clients. The first Thresholds Health center will be located in Chicago's Austin community area. We know the the integration of primary, mental health and substance use care is a proven approach to address healthcare needs and we look forward to serving patients with our signature person-centered care.
