Neuropraxis is a home and community brain and spinal cord injury rehabilitation program serving rural and urban areas in California. Our mission is to provide a comprehensive and customized treatment program through evidence-based practices facilitating the transition of individuals with a brain or spinal cord injury to a life of purpose, social connection and optimism. Our goals are to: Ease navigation during transition of care Optimize outcomes through innovative practices Prevent complications and readmissions Establish a sustainable community support Achieve autonomy and wellness through a collaborative process, education and advocacy At Neuropraxis, we believe that life is meant to be lived and each participant has the right to a healthy, joyful, and thriving life once they return home as it's the cornerstone to the reconstruction of a coherent self, regaining a sense of autonomy and finding hope in life after a brain or spinal cord injury.
