Care Choice Health Systems, Inc. provides Care Management and non-medical home care services to San Diego County. Our Mission is to provide high quality Care Management and caregiving services to our community that maximizes the quality of life for our customers by providing ethical and compassionate care. Our philosophy is to offer personalized care for each patient providing them the support they need to stay in their home or place of residence. Our target population is primarily people with compromised mobility, physical limitations, cognitive disorders and personal care needs.
