Health Bound Health Network is an accredited, multidisciplinary clinic and resource center that serves over 45 communities across Ontario. Our interdisciplinary team of healthcare providers is committed to providing evidence-based and client-centered care at our state-of-the-art facilities, in our clients' homes, and hospitals. We offer a wide variety of healthcare programs such as multidisciplinary rehabilitation services, hospital discharge planning, and case management for patients with serious injuries. We also have a province-wide outreach, rapid response time, and technologically advanced equipment to help you in your journey to recovery. Health Bounds programs and services are for everyone - individuals, community, corporate and government entities, hospitals, and physicians.
