Combining nearly 150 years of service to our region, Community Wellness Partners will remain a faith-based 501 (c)(3) non-profit. Our affiliation creates the most comprehensive post-acute continuum of services in Oneida County. Our programs range from, but are not limited to, Independent Living, Home Care, Adult Daycare, Assisted Living, Rehabilitation and Skilled Nursing. Our goal is to guide you on your journey through our broad array of health services spanning all levels of intensity of care.
